At the transitions between hospital and home failures frequently occur that can create challenges
in the interaction process. The purpose of this study was to describe nurses' experiences, both in
hospital and community health care, at the discharge of persons with cancer in palliative phase to
their own home. A qualitative descriptive design using open focus group interview was selected for
the study. Participants were recruited from a local hospital and three municipalities. Five interviews
were conducted with four participants in each group. The interviews were analyzed by qualitative
content analysis. Nurses' experiences were described through two categories: common meeting area
and understanding of each other's work. Nurses in both service levels expressed that a common
meeting before discharge was of great importance for the transmission of the palliative care and
continuity of care. Coordinators of cancer and oncology nurses in the community health care prove to
be central staff. There was a lack of knowledge about the organization of the health care and between
service levels. The results revealed that there is a potential for improvement in creating good
procedures at discharge to clarify expectations, predictability and continuity of care. The importance
of results of clinical practice may be increased focus, reflection and development of interaction
between hospital and community health care.
