Background: Research focusing on the transfer of patients from hospital to community health
care is important.
Aim: To shed light on nursing students’ experiences of how they practice the information exchange
between the hospital and community health care in connection with the transfer of patients, and how
these are received and followed up by nurses in community health care the first days after arrival.
Method: The study has a qualitative design. Individual interviews were conducted with ten students
from a bachelor’s education in nursing. Data were analyzed using manifest and latent content
analysis.
Findings: The written nursing summary from the hospital could be inadequate. The time for the
patient’s arrival was unpredictable for both the patient and the community nurse. Both the reception
and follow-up of patients in community health care could be characterized by nurses who signaled
time pressure, but also by nurses who took their time with the patients.
Conclusion: The coordination between hospital and community health care needs greater focus, and
guidelines must be developed to ensure good patient pathways
