Heart failure, here confined to chronic,
left ventricular systolic dysfunction, is a
progressive disease associated with a
high degree of mortality. Besides lifestyle
modification, medical treatment
is essential. The optimalization of the
heart failure patient’s drug regimen
takes time, and is often taken care of
by nurses and cardiologists in specialized
out-patient clinics. The goals
of heart failure therapy are symptomatic
improvement and ultimately
a reduction in the risk of morbidity,
rate of hospitalization, and mortality.
The typical heart failure patient is >
65 years old and have heart failure
secondary to ischemic heart disease
and/or atrial fibrillation. In addition
to standard heart failure treatment with
beta blockers, ACE-inhibitors, diuretics
and aldosterone antagonists, additional
drugs like statins, platelet inhibitors
and anticoagulants are often indicated.
Many patients have other diseases like
diabetes or COPD as well, contributing
to long and complex drug-lists. The aim
of this article is to give a summary of
standard drugs used in the treatment of
heart failure due to systolic dysfunction,
based on medical textbooks, published
clinical guidelines and articles, as well
as experiences from practice on a heart
failure out-patient clinic. The role of the
pharmacist as a care giver for these
patients is outlined.
