Sammanfattning
The project comprises in-patients who undergo post-operative intravesical Mitomycin bladder
flushing following transurethral removal of a staged superficial bladder tumour.
Patients commonly describe discomfort of stinging and pain in response to the therapy. The aim of
the project was to investigate whether a change in installation method might reduce the degree of
discomfort experienced and thereby increase patient compliance.
The study was an open, prospective, randomised controlled trial. 100 patients were assigned to
either an intervention group or a control group, and all patients answered questions concerning
discomfort in a questionnaire.
In the control group, the patients had the Mitomycin infusion installed via a bladder catheter, and the
catheter was clamped for the two hours’ duration of the therapy. In the intervention group, the bladder
catheter was open and the urine bag was elevated one metre above bladder level according to the
water lock principle.
The hypothesis was that bladder contents during bladder cramping would be able to flow out of the
bladder in the absence of counter-pressure. The hydrostatic pressure resulted in fluid-backflow into
the bladder.
The intervention group experienced less discomfort. Urinary urgency was significantly reduced. The
patients experienced reduced seepage along the bladder catheter, and 14 per cent more individuals
in the intervention group completed the therapy.
