Sammanfattning

Background and aim: To cope with daily activities after discharge from hospital may represent several challenges for older people. The aim of this study was to illuminate occupational therapists’ experiences of promoting activities for older patients who are in transition from hospital to own home. Method: A qualitative design was used. Eight occupational therapists, four working in hospitals and four working in community home services, were interviewed. Qualitative content analysis was used in the analysis of the transcribed interview texts. Findings: Three main categories emerged from the interviews: «to facilitate for activities and coping on an individual level», «potentials in the patient’s physical and social environments» and «interaction between service levels». A main theme was «to find a balance between patients’ needs and organizational considerations in vulnerable and complex transitions». Conclusion: Occupational therapists are aware of challenges, and potentials, to promote coping in daily activities for older people discharged from hospital to own home.
