The purpose of this study was to explore the challenges, assessments and measures
experienced nurses faced in survey of postoperative pain in adult patients without the ability to self-
report. Examples of patients with reduced ability to self-report are elderly with cognitive impairments,
people with mental retardation, unconscious patients and patients who are critically or terminally ill.
Research has had limited focus on how experienced nurses assess and judge postoperative pain in
the patient population. The study was a qualitative interview study with an interpretative approach.
Focus group interviews were used so that experienced nurses could describe and share their
knowledge and experience with assessment and judgement of postoperative pain in this group of
patients. The sample consisted of nine nurses with an average of 13 years experience. This study
found that experienced nurses primarily observed and interpreted changes in the patients’
physiological parameters. Secondly, pain behaviour was assessed. No pain assessment tools were
used. This study showed that experienced nurses were uncertain about the quality of both the
assessment and the evaluation of pain treatment given to patients. There is a need to develop and
implement guidelines for safe pain treatment for patients with reduced ability to self report in
postoperative wards.
