Background: The diagnosis of Irritable Bowel Syndrome (IBS) is based on typical symptoms
after exclusion of other disorders. There is a strong need for a biomarker that confirms the diagnosis.
The paper reviews available biomarkers for IBS.
Material and methods: The paper is based on a review of the literature, the author’s research in the
field and experience with patients with IBS.
Results: A large number of potential biomarkers based on tests and examination of blood, gut
function, feces, the gut wall, cerebral functions and psychological disorders have been published
during the last years. This research has resulted in new and valuable knowledge about IBS, but no
suitable biomarker for use in daily practice.
Discussion: The main reason for not finding one biomarker that confirms the diagnosis of IBS is that
IBS probably consists of several disorders with different etiology and pathogenesis which are
presented with similar symptoms. Hopefully, further research will result in a biomarker based on
typical symptoms that together with a combination of a few biomarkers confirms the diagnosis.
