Families affected by severe stroke experienced by one of their close relatives face extensive life
changes. The aim of this phenomenological hermeneutic study was to explore the long-term
changes in family life after stroke, by means of a meta-synthesis of three studies. Thirty-nine
narrative interviews with stroke survivors, their spouses and adult children highlighted three
phenomena of paramount importance in the daily lives of these families; 1) communication,
2) relationships and 3) identity. Difficulties in communication arose due to aphasia or other
cognitive and behavioral changes post-stroke and due to the reactions of the family members
to these changes and their reluctance to share thoughts and emotions openly. The burden of
caregiving, loss of roles, social network and previous enjoyable family interactions challenged
the concept of self in all parties. Families with strong foundations were most capable of tackling
these changes. Interventions such as psychosocial support and professional guidance
may benefit these families, as there is an expressed desire to restore family bonds and intimacy,
and the concept of self in each family member.
