Background: To reduce the growth of public expenditure, generic substitution of drugs was implemented in Norway in 2001. The aim of this study was to examine the extent and costs of reservations against generic substitution, and investigate whether price affects drug version choice.
Material and methods: An extensive dataset containing all prescriptions dispensed in 2016 on 116 different chemical substances was obtained from the Norwegian Pharmacy Association. The drugs in the dataset had a total sales value of 3.25 billion NOK.
Results: Reservation by physicians was found on 7% of the prescriptions, and reservation by patients on 14%, making a total of 21%. The calculated costs due to reservations were 401 million NOK, where 230 million was paid by the Norwegian National Insurance Scheme and 171 million by the patients. The price difference between the brand and the generic versions had a negative correlation with patient reservation, and positive correlation with physician reservation.
Conclusion: One fifth of dispensed prescriptions contain reservations against generic substitution, and the costs of the reservations amounts to approximately 12% of the sales value of the drugs. Physician reservation increases and patient reservation decreases with increasing drug prices.
