Of all deaths in Norway in 2016, 50 percent took place in nursing
homes, and 30 percent in hospitals. Patients are often
not asked about their wishes for end-of-life care, resulting
in poor decision-making and often unnecessary hospitalizations.
The aim of this study was to improve the understanding
of the challenges met by nurses in nursing home patients’
last days of life.
We conducted six in-depth interviews with nurses in a
Norwegian municipality. The nurses worked in three nursing
homes of different sizes. Both experienced nurses (more than
ten years of experience) and newly graduated (less than two
years of experience) were included in the study.
The main result of this study was that nurses in nursing
homes have complex and diverse duties in the last days
of the patient’s life, including heavy professional responsibilities.
The interviewees recommended that advance care planning
(ACP) was made part of the clinical routine. The ACP
conversation should involve the patient, next of kin and
attending nurse and physician. The results demonstrate a
need for more certified nurses in nursing homes.
