Coronectomy is a treatment option for mandibular third molars when there is an indication for removal, and the risk of an inferior alveolar nerve (IAN) injury is considered high. The procedure has gained increasing attention and with the most recent follow-up studies, the long-term morbidity is explained.
There is a general agreement that coronectomy is associated with a low risk of IAN injuries and other postoperative morbidity. However, coronectomy is not without potential complications and should only be performed when an intimate relation to the IAN is present. If this is not the case, complete removal
is recommended. Furthermore, coronectomy is surgically a challenging procedure where a number of steps needs to be fulfilled in order to obtain a predictable result. The purpose of this article is to highlight the most important key factors to perform a coronectomy successfully based on the newest available evidence. Additionally, the purpose is to give recommendations regarding the information to the patient and advise a radiological protocol for the postoperative control period.
