Obstructive sleep apnea (OSA) in adult patients can be treated with a mandibular advancement device (MAD). OSA is diagnosed by medical consultants, but MAD is indicated as the first-choice treatment in patients with mild to moderate OSA and in patients with severe OSA who do not accept therapy with CPAP (Continuous Positive Airway Pressure). MAD works by increasing the upper airway dimensions. Thus, the respiratory stops, which are caused by collapse of the upper airway, are reduced. Though MAD is inferior to CPAP with regards to decreasing the number of respiratory stops, patients prefer sleeping with MAD rather than with CPAP. The short-term side effects of MAD treatment are minor and most often transient, but over time there may be unintentional permanent changes in the occlusion with unwanted reduction of the overjet and overbite. As a consequence, MAD therapy requires both multidisciplinary collaboration and experienced dentists for sufficient and efficient treatment of OSA patients.
