The detection of early caries lesions is crucial to enable secondary prevention and non-invasive management of the disease. This is especially true for occlusal fissures where dentin lesions may be covered by apparently intact enamel and various technologies have been introduced as adjuncts to visual-tactile examination and bitewing radiographs. The methods are based on either optical fluorescence, for example DIAGNOdent, or electrical resistance (CarieScan Pro). The methods are in general accurate in detecting advanced occlusal lesions (>1/3 of dentine) and the risk for false positive diagnosis is low. The ability to correctly identify the early stages of lesions is, however, significantly impaired with an increased number of false positive tests. There is insufficient information to conclude how the use of adjunct diagnostic tools affects the clinical decision-making in general practice, and studies on cost-effectiveness are lacking. The DIAGNOdent and CarieScan Pro devices may be used selectively as a second opinion to the regular examination, in particular when bitewing radiographs are not available.
