A 25-year-old female with an ankylosed maxillary central incisor was referred to our department by her orthodontist for evaluation and treatment. Segmental osteotomy was performed under per oral sedation and local anesthesia. A full-thickness mucoperiostal flap was elevated and the osteotomy was completed with a piezoelectric micro-saw. The dento-osseous segment was mobilized with a chisel osteotome and wired to her orthodontic appliance for one week. A distraction period of 5 weeks and a consolidation period of 6 weeks followed after surgery. Total treatment time was 3 months. No complications were seen. Radiographic and clinical follow-up 2 and 4 months after the surgery revealed satisfactory healing, and a good functional and esthetic result. Segmental osteotomy with distraction osteogenesis in the treatment of ankylosed teeht is not well documented and there are few publications on the subject. Movement of an ankylosed tooth to the final position can be difficult to complete with surgery alone. Distraction of the dento-osseous segment can facilitate refinement of tooth position and also enable stretching of the soft tissue.
