Prosthetic rehabilitation with implants in the posterior part of the maxilla is frequently compromised due to pneumatisation of the maxillary sinus and reduced vertical bone height after tooth loss or congenitally missing teeth. Implant treatment within this region requires, as implant treatment involving all other regions, meticulous treatment planning based on a thorough patient’s history as well as a clinical and radiological evaluation to perform a predictable long-lasting implant treatment with minimal risk of complications. The patient’s history must clarify the cause of tooth loss and the patient's general health status, allergies, tobacco and medicine consumption, financial capacity as well as request and expectations for the implant treatment. The clinical and radiological evaluation includes assessment of the alveolar ridge and evaluation of the need for alveolar ridge augmentation before or in conjunction with implant placement. The treatment plan must be patient specific and based on scientific evidence, thus ensuring the most predictable and optimal implant treatment with the lowest possible risk of complications. In the present review, the principles of a meticulous treatment planning prior to implant placement in the posterior part of the maxilla will be presented with focus on aspects specific for this region.
