Patients with borderline difficulties
have a great potential for regression.
For that reason contemporary
treatment is mainly performed outside
institution. However, especially
in selfdestructive periods, some
patients will need to be institutionalized.
The patient’s self-mutilating
behaviour constitutes a nursing
challenge. The aim of this study is to
learn how nursing can restrict
and/or promote regression in patients
who harm themselves.
Orem’s theory of self-care deficit is
chosen as nursing theory. In addition,
texts about borderline difficulties
and self-mutilation and nursing
litterature on borderline patients
who harm themselves are applied.
Qualitative research interviews are
used in the empirical part of the
study. 12 nurses with experience
and interest in the problem were
interviewed. The analysis is based
on a hermeneutical/dialectic approach,
and various techniques are
used to create meaning and structure.
The respondents show a great variety
of emotional reactions to selfdestructive
behaviour. These reactions
vary from anger and helplessness to
sympathy and compassion. The
power of these emotions makes it
tempting to understand some of
them in light of countertransference.
Emotions influence what nurses do
to help patients. The results of this
study are discussed along two
dimentions; responsibility and cooperation.
By crossing the dimentions
four positions representing various
ways of meeting patients are produced.
A simple model is presented
that proposes how nurses can help
self-mutilating patients.
