Introduction: The article evaluates the postoperative results of breast reconstruction with the free TRAM flap. Materials and methods: All patients undergoing breast reconstruction with free TRAM flaps in our institution between June 1994 and December 2000 were reviewed. A total of 86 free TRAM flaps were performed in 84 patients. The results were compared to those of other plastic surgery centers. Results: Total flap loss occurred in 5 of 86 reconstructions (5.8%). The complications were related to the flap, the abdomen or other causes. Partial flap loss or necrosis occurred in 5 patients (5.8%). Hernia occurred in 7 patients (8.3%). The results presented in this article suggest a tendency to more complications in patients who smoked or were overweight. Discussion: The results are equivalent to those of other plastic surgery centers. A breast reconstruction with a free TRAM flap is an operation with a potential risk of various complications, and therefore the patients must be carefully selected. Despite the complications, the free TRAM flap is a good method of breast reconstruction.
