Introduction: Local intraarterial thrombolysis has in university hospitals been shown to be an effective treatment in the management of acute limb ischemia. The purpose was to evaluate how the treatment works in a provincial hospital. Material and methods: During a period of 21 months 21 extremities with acute or subacute ischemia were treated with local intraarteriel infusion of tissue plasminogen activator. The extremities were evaluated angiographically and if found technically not suitable for vascular surgery thrombolysis was initated. Results: At 30 days follow-up the limb survival was 85%, and after two years it was 75%. Five extremities were amputated, three within 30 days. Three patients died. One patient had a local bleeding and sepsis developed in one patient. The sepsis was caused by an infection at the site of catheterization. Discussion: Intraarteriel infusion of tissue plasminogen activator seems to be an efficient and gentle therapy, which can be performed with low mortality and few side effects.
