Introduction: The statistical material of the Institute of Patient Insurance has significantly fewer reported claims than earlier studies of treatment injury in Denmark. We have examined the incidence of treatment injuries and "near misses'' in a urological department during the course of one year. For this study a modification of the Swedish "St. Maria'' classification was used. Materials and methods: Two specialists read 1,573 files including 2,012 cases of patients discharged in 1997 and evalu-ated the standard of treatment. Results: Of the cases examined, 1,803 could be acquitted of treatment injury. Among these were 122 "near misses''. There were 209 cases of treatment injury. Of these 150 were trivial, 41 were light, nine were severe, six were very severe and three were fatal, according to the classification. Discussion: Departments of urology report about 30 cases each year to the Institute of Patient Insurance. Of these, two are reported by the investigated department. Had all 209 cases of treatment injury been reported, and had these reports been handled according to the usual standards of the institute, it would have resulted in an estimated compensation of 12 million Danish crowns (approximately $1.7 million). The num-ber of fatal injuries was 4 (0.2%). More studies are needed before it is possible to ascertain with any degree of certainty the incidence of treatment injury in Denmark.
