Introduction: Our aim was to assess the frequency of pyogenic arthritis after arthroscopic knee surgery in Denmark and if possible to find common circumstances that could motivate a change of procedures in order to lower the rate of infection. Materials and methods: A retrospective study including 43 cases reported to the Patient Insurance Association in Denmark in the period 1998-2000. Information was also obtained from the National Patient Registry as well as from questionnaires sent to 63 orthopedic departments in Denmark. Results: The infection rate was found to be 0.1%. In 67% of the reported cases, a positive bacteriological diagnosis was made by the analysis of joint fluid or synovial tissue. Staphylococci species were isolated in 90% of the cultured microorganisms. Approximately 80% of the infections were caused by simple arthroscopic surgery. All patients were treated with synovectomy and long-term antibiotic treatment. Discussion: The actual rate of infection can in fact be higher than the 0.1% found, as the number of unreported cases can be as much as 100 times as high. Theoretically, the use of preoperative onetime prophylactic antibiotics in arthroscopic knee surgery can be socioeconomically beneficial.
