Coeliac disease (CD) is present in 1% of the population, and serological screening of subclinical cases is increasingly important. Comparative studies of assays for IgA tissue transglutaminase antibodies (TTGA), IgA endomysium antibodies (EA), and gliadin antibodies (GA) show that human recombinant IgA-TTGA tests have higher sensitivity and specificity than guinea pigbased assays, and that human TTGA and EA are highly specific. The sensitivity of CD screening is improved by combining TTGA with EA. GA and IgG-based assays have no clearly defined role.
