Introduction: Based on the varying results of esophageal resections in Denmark, the aim of this study was to compare the cost-effectiveness of the present organisation with alternative means of organisation. Materials and methods: The analysis was based on the results of a nationwide analysis of all esophageal resections in the period 1997-2000, together with a separate analysis from 2000-2002 performed at one center organised into one large region as well as into multidisciplinary teams. The costs reported are the direct costs, and the effect is the probability of a succesful intervention. Results: The average cost of a decentralised treatment in 1997-2000 was 394,294 DKK, compared to 223,915 DKK for treatments carried out at the regional center. Discussion: This analysis demonstrates that esophageal resection can be performed at lower cost when carried out in departments organised into multidisciplinary teams and with a high volume of operations performed.
