We report on recurrent group B streptococcal (GBS) infection in two infants with severe meningitis and a twin with milder meningitis. Our data suggest that, in severe GBS meningitis, a minimum of three weeks treatment with high-dose antibiotics – i.e. penicillin 400,000 to 500,000 IU/kg/day after initial treatment with ampicillin 400 mg/kg/day and aminoglycoside – is necessary to avoid relapse. After three weeks of treatment a lumbar puncture should be performed.
