Chronic heart failure (CHF) is a frequently appearing clinical syndrome with a high morbidity and mortality rate despite improved treatment modalities. Patients with this condition have symptoms of dyspnoea and fatigue that often limit their daily activities. The pathophysiological explanation of these symptoms has been expounded by first a cardiovascular and later a neurohormonal model, focusing on the reduced left ventricular function. While this has contributed to the comprehension of CHF, it has also, to a lesser extent, focused on the myopathy of the skeletal muscle being of major importance. The myopathy of CHF can activate receptors in the skeletal muscle that are sensitive to particular metabolic changes, and activation of these receptors can induce increased ventilation, fatigue upon exertion and increased activity of the sympathetic nervous system. Exercise as a treatment modality has been shown to affect many of the peripheral changes, specifically to normalize the function of the skeletal muscle. Furthermore, exercise reduces dyspnea and fatigue, improves the patient's quality of life and has recently been shown to increase the survival rate of the disease.
