Foetomaternal transfusion in pregnancy usually occures unexpectedly and may result in foetal death. In this case we describe decreased foetal movements for five days during the 34th week. A non-stress test showed decelerations and decreased beat-to-beat variability. Caesarean section was performed on an emergency basis. The infant had an initial hemoglobin concentration of 1.9 mmol/L but did well after transfusion therapy. The foetomaternal transfusion was estimated to have been 175 ml of foetal blood. This case demonstrates that daily counting of foetal movements is of vital importance.
