Several studies have shown that between one quarter and one third of patients diagnosed with epilepsy do not actually have epilepsy. The episodes most often misdiagnosed as epilepsy are syncope and psychogenic nonepileptic seizures, as these episodes may display symptoms with motor activity that can mimic epileptic seizures. A detailed clinical history is the most important factor when diagnosing epilepsy. Repeated EEGs support the clinical diagnosis in the majority of patients with epilepsy. Additional tests, e.g. tilt table test and video EEG telemetry, are useful in difficult cases.
