The prevalence of epilepsy is approximately 5 in 1,000 among people of reproductive age. Fertility rates are reduced in people with epilepsy; this seems to be the result of a combination of psychosocial and biological factors. Sexual dysfunction is common. Antiepileptic drugs affect the concentrations of the sex hormones. Menstrual disturbances, polycystic ovaries and reduced semen quality are more common in women and men with epilepsy, respectively. Enzyme-inducing antiepileptic drugs increase the metabolism of oral contraceptives. Treatment with antiepileptic drugs increases the rate of major malformations by two to three times in children of mothers with epilepsy. This paper presents recommendations for the management of fertile women with epilepsy.
