Parastomal hernia is a frequent complication after formation of an ileostomy or colostomy, with a reported incidence of up to 48%. Symptoms can be mild without the need for surgical intervention. Generally, it is recommended to adopt a conservative approach, saving surgery for the cases where there is severe chronic or intermittent pain, refractory skin problems or psychological problems because of aesthetics. Surgical results are highly variable, with reported recurrence rates of 0-100%. We discuss the literature in this field, with special emphasis on prophylactic measures during the primary operation (such as placing a mesh at the primary stoma formation) and laparoscopic mesh repair, on which there are promising casuistic reports and which probably has the usual advantages of minimal invasive surgery and low (none reported so far) recurrence rates.
