Approximately 200,000 Danes suffer from COPD, and acute exacerbation of this condition is responsible for about 23,000 admissions to hospital every year. The cornerstones of the standard medical treatment (SMT) for COPD patients admitted to hospital are oxygen, corticosteroids, bronchodilators, and antibiotics. By adding non-invasive ventilation (NIV) to the SMT for selected patients with acute respiratory failure, the mortality rate is reduced from 22% to 9%, resulting in a number needed to treat (NNT) of 8 (6-13). The risk of intubation is reduced from 33% to 14% (NNT = 5 [4-7]). A cost-effectiveness analysis showed that NIV is cheaper and more effective than SMT alone. It is estimated that adding NIV to the SMT for patients with exacerbation of COPD and acute respiratory failure may reduce the number of deaths by 230 and intubations by 400 in Denmark each year.
