The effect of rehabilitation in chronic conditions and for some diseases the value of hospital-based rehabilitation at home is assessed. Rehabilitation includes optimised medical treatment, disease-specific interventions, patient education, physical exercise, tobacco and alcohol intervention and nutritional advice. Evidence for the effect of rehabilitation is found in patients with ischemic heart disease, heart failure, type 2 diabetes and stroke and in those with hip fracture and complex geriatric conditions. Hospital-based rehabilitation and home care service are effective in patients with heart failure, stroke, hip fracture and complex geriatric conditions.
