Introduction: Rectal prolapse is the full-thickness prolapse of the rectum through the anal sphincter mechanism. This condition is seen mainly in elderly females. The condition can be treated either by an intra-abdominal approach or by the less extensive perineal approach. Method: We evaluated the results of all patients undergoing perineal rectosigmoidectomy ad modum Altemeier during the period January 1995-October 1999. Results: In all, 24 patients (mean age 83.5 years) underwent the operation. Twenty-three of the patients were studied, with a mean follow-up period of 16 months. Postoperative complications were seen in one patient (4.2%). The recurrence rate was 27%. Discussion: We conclude that treatment of rectal prolapse by perineal rectosigmoidectomy is safe, especially when treating elderly patients with concurrent diseases. The postoperative complication rate is low, but the recurrence rate is high compared to that of the intra-abdominal approach. In case of recurrence, the perineal rectosigmoidectomy may be repeated.
