Introduction: The aim of this study was to investigate the findings, data and advice related to diagnosis of an infection before antibiotic treatment, as well as any later changes in treatment. Materials and methods: The study was carried out as a period prevalence study in four departments of Rigshospitalet, Copenhagen, where there was frequent use of antibiotics. The study included 149 patients. Data collection was based on patients' records and nurses' medication records. Results: The use of clinical and laboratory findings differed among the four departments in relation to the initial antibiotic treatment, and differences were found among the departments in the use of antibiotics and the frequency of culturing. In all, 219 changes of treatment were registered for the 149 patients. The neurointensive and intensive care units consulted the clinical microbiology department in half of the cases when antibiotic treatment was changed, whereas the haematological department rarely did so. Discussion: Antibiotic treatment strategies differed among the departments. Those having frequent conferences with the clinical microbiology department had a higher culturing frequency and level of diagnostic precision regarding the bacterial agent, the laboratory findings were used more often as diagnostic tools, and the initial antibiotic therapy was changed less often.
