A 52-year-old woman was admitted showing clinical and biochemical signs of hypoxia due to severe anaemia (haemoglobin 1.1 mmol/L) caused by an ulcerating borderline gastrointestinal stromal tumour. After initial treatment with fluid resuscitation and blood transfusions, all symptoms and signs resolved when the patient's haemoglobin concentration rose above 3 mmol/L. This demonstrates that, given time to adapt, it is possible, at least for a shorter time, to survive very low haemoglobin concentrations. At admission the patient's estimated oxygen delivery was 2.6-3.6 mL O2 /kg/min, supporting the earlier estimation of basal oxygen need.
