We report two cases of traumatic dissection and occlusion of the internal carotid artery after blunt head and neck injury. A diagnosis was made using MR and MR angiography, and any resulting brain ischemia was easily detected if present. Interestingly, imaging of the circulus arteriosus Willisii illustrated well the influence of anatomical variations in this anastomosing system on the risk of brain tissue ischemia following internal carotid dissection and occlusion. However, the symptoms and clinical signs of internal carotid dissection might be subtle, and the condition remains underdiagnosed.
