Kirurgisk behandling af svaer fedme. Effekt af laparoskopisk justerbar gastrisk banding
Sammanfattning
Introduction: Our goal was to determine the outcome of obesity surgery (laparoscopic adjustable gastric banding, or LAGB) on weight loss, risk factors, quality-of-life measures and complications in the first 55 obese subjects treated with this method at Aarhus University Hospital. Results: The patients' average initial body weight was 134 kg; after two years, their average weight loss was 16 kg. The patients' BMI was reduced from 46 kg × m-2 to 39.5 46 kg ×m-2 ; their blood glucose levels were reduced and their HDL cholesterol level increased, but no changes were observed in total cholesterol or blood pressure. Overall, 22% of the patients needed reoperation within the two-year period because of problems (infection, leakage or pain), but for the first 18 patients the surgical morbidity was 39% whereas for the last 18 patients this figure was reduced to 6%. Despite these problems, the patients were rather satisfied with the operation and reported a 64% improvement in their quality-of-life self-assessment. Discussion: LABG is a safe procedure which effectively induces a marked and sustained body weight loss with a reduction of risk factors. This surgical procedure particularly requires a skilled surgeon, and therefore we recommend that this procedure be done only at centralized centers.
