Introduction: The aim of this study was to investigate the effectiveness of oral transmucosal fentanyl (OTF) for painful procedures in children in a hospital accident and emergency (A & E) department. Materials and methods: Thirty children presenting in the A & E with extremity fracture (11 children) or soft tissue injury (19 children). The median age was 5 years (range 3-12 years). The painful procedure was suture of wound (8 children), injection of local anaesthesia (11 children) and application of a plaster cast (11 children). The patients received OTF containing 8-15 mikrogram/kg of fentanyl. Pain severity scores during the painful procedure were assessed using the Oucher Scale (0-100). Any side effects were noted. Results: The median pain score during the painful procedure was 10 (range 0-100). The incidence of nausea and vomiting was 27%. One child experienced a decrease in oxygen saturation to 95%. There was no clinically significant respiratory depression or severe sedation. Discussion: OTF may be particulary useful as an analgesic in children presenting to the A & E following trauma. Monitoring respiratory function is mandatory. The most common problem is frequent incidence of nausea and vomiting.
