Sammanfattning

Background. In Sweden county councils run hospitals, and the local authorities are responsible for home care of the elderly. Whilst the quality of hospital and home care is generally good, there are quality problems when transferring elderly people from hospital to home care. Purpose and theory. This paper considers these interagency quality problems as they emerge in discharge meetings and enlightens the problems that arise from the perspective of service management and communications theory. Method. A qualitative study was undertaken with nurses and aid-assessors from three Swedish hospitals and three local authorities. Results. It presents findings from discharge meetings, which show the different cultures, and assumptions of the two care systems and illuminate the issues of communication and understanding, which underlie the problems. Interpretive discussion and conclusion. The paper considers how discharge meetings could be improved by paying attention to the environment and communication aspects of the meeting.
