Medication lists for 89 patients cared for by the district nurse were compared with GP's lists within the same municipality. Discrepancies were uncovered and analyses were performed to ascertain, how they had occurred, and which factors might have an influence on their number.
Results. Discrepancies in either drug or daily dose were found in 57% of patient cases. These discrepancies mainly occurred in connection with the GP's prescriptions or when the patient was being admitted to hospital. The majority of discrepancies were found within cardio-vascular medication. Each patient was given an average of more than six drugs. Despite discrepancies in 51 medication lists, the registration made by the district nurse was wrong in only 12 of the cases when compared to the physician's actual prescription.
Interpretation. Discrepancies could result in the patient not being given the prescribed medication or alternatively being given more drugs than they need. This affects not only the patient, but also has socio-economic effects and is unsatisfactory from a nursing point of view. Quality development of the area is required.
