The environment of any intensive care ward is characterised by technology and a high level of activity 24 hours a day. The senses of critically ill patients are over-stimulated. Because of their illness and medication, it is very difficult for critically ill patients to make sense of the technological environment, and patients have difficulty distinguishing between fact and delusion. Following their discharge from intensive care, many patients suffer from delusions, nightmares and flashbacks.
A diary of patients' stay in intensive care allows patients and their families to reconstruct and understand the course of events. The diary is not a medical record, and care staff use narrative everyday language free of jargon. It is the responsibility of the staff to ensure that entries are made to the diary and relatives are encouraged to commit their thoughts to paper.
Clinical experience and the results of surveys show that a diary is an important tool when it comes to patients and relatives dealing with a difficult time.
