Posttransplant diabetes (PTDM) is persistent hyperglycemia following organ transplantation and is a serious and frequent complication. PTDM increases the risk of graft failure and cardiovascular disease. The most importance factor is the posttransplant immunosuppressive treatment. PTDM develops when insulin resistance is not compensated by secretory hyperinsulinemia. New immunosuppressive regimes seem promising in reducing PTDM. Guidelines recommend pre- transplantation OGTT, early diagnose of PTDM and individualized immunosuppressive treatment. Antidiabetic treatment should be initiated early.
