Periodontal health is of paramount importance for all teeth, both sound and restored. When the clinical crowns of teeth are dimensionally inadequate, it is hard to accomplish an esthetically and biologically acceptable restoration. Crown lengthening is a periodontal resective procedure, aiming at removing supporting periodontal structure to gain sound tooth structure above the alveolar crest level. The crown lengthening procedure can be limited to the soft tissues when there is enough gingiva coronal to the alveolar bone. However, the majority of cases will involve bone recontouring as well as gingival resection to accommodate esthetic and functional demands. This article reviews the concept of biologic width, indications and contraindications for crown lengthening, and surgical techniques. Four cases are presented utilizing crown lengthening as an integral part of treatment to restore teeth and to correct compromised esthetic smile.
