Health care-related infections, especially pneumonia in ventilator-treated patients (VAP), are often caused by bacteria resistant to many antibiotics. Adequate antibiotic treatment is of great importance for these patients. VAP is therefore one reason for the great use of broad-spectrum antibiotics within intensive care units, contributing to further selection and spread of antibiotic-resistant bacteria. Quantitative lower respiratory tract cultures can be used to discriminate infection from colonization. They can thus be used to avoid both inadequate and unnecessary broad antibiotic treatment and also to avoid treatment of patients without infection. The power to discriminate between infection and colonization is however not 100%. Optimal use of quantitative cultures requires standardization of sampling techniques and knowledge of what thresholds that applies for each technique.
