In this overview of Chlamydia trachomatis (C. trachomatis) and Mycoplasma genitalium (M. genitalium) infections in women, epidemiological and clinical aspects will be discussed. C. trachomatis infections are still a major challenge despite opportunistic screening and contact tracing performed in Sweden over the last two decades. Studies from Malmö, Sweden, have shown an increase in complications such as ectopic pregnancy and chlamydial conjunctivitis over the last decade. C. trachomatis remains a major pathogen in pelvic inflammatory disease (PID), found in about 1/3 of all cases of PID. In women, the new genetic variant of C. trachomatis causes similar symptoms as the ordinary wild types of C. trachomatis but in a lower proportion of cases. M. genitalium has emerged as an important pathogen with manifestations similar to C. trachomatis although the symptoms tend to be milder. M. genitalium infection has been associated with PID in only a few reported cases and its role in PID in western countries is still unknown. In a recent study from Malmö, Sweden, M. genitalium was strongly associated with PID in women undergoing surgical termination of pregnancy (OR 6.29; CI 1.56-25. 2).
