Autoimmune liver disease is a dynamic process that may appear as variant forms of the classical types or change in character over time. Overlap syndromes are cholestatic immune mediated diseases that fulfil criteria for two classical autoimmune liver diseases at the same time or consecutively. Most common are PBC/AIH and AIH/PSC. They should be suspected in a patient who has or develops a combined hepatocellular/cholestatic LFT profile and/or different types of autoantibodies. Efficient immune suppressive therapy can be directed at the AIH component but conclusive studies regarding therapy in these patients are still lacking.
