Vitamin D deficiency in chronic kidney disease is an established risk factor for cardiovascular disease. At the same time, several epidemiological studies have shown that a large proportion of the population have vitamin-D levels below current recommendations.
Activation of vitamin D is reduced in patients with chronic kidney disease (CKD) due to increased expression of fibroblast growth factor-23, a phosphaturic hormone with anti-vitamin D properties. This causes a vicious cycle of disturbances in mineral metabolism and development of secondary hyperparathyroidism.
Vitamin D substitution is associated with reduced mortality in patients with advanced CKD and secondary hyperparathyroidism. However, beneficial effects of such treatment in earlier CKD and in patients with normal renal function remain to be proven.
