In Sweden, 200–300 cases of infection with enterohemorrhagic Escherichia coli (EHEC) are reported each year, about 60 per cent are domestic cases. Ruminants, especially cattle, are considered to be the main reservoir, and EHEC O157 has been detected in about 10 per cent of Swedish dairy herds. The human infection is typically characterized by acute bloody diarrhea, abdominal pain and low fever, but the symptoms may vary. Hemolytic uremic syndrome (HUS) may occur in 5 to 10 per cent of the cases, mostly in infants, but also in adults. We describe a fiftysix year old woman, who four days after onset of bloody diarrhea developed ischemic colitis requiring colectomy, HUS and also neurological manifestations. She was treated with dialysis and recovered fully after several months. A couple of days before onset of disease she had tasted raw minced meat. Identical strains of EHEC O157 were isolated from the stools of the patient and the minced meat.
