Sammanfattning

New classes of potent anticoagulant drugs and platelet inhibitors are increasingly used for primary and secondary prophylaxis in cardiovascular disease and after coronary interventions. As clinical experience with these new drugs still is limited, guidelines how to handle the increased risk of severe bleeding in connection to planned surgical procedures are urgently needed. In this literature study we have summarized the clinical evidence and prepared an evidence-based recommendation on how to deal with anticoagulant drugs preoperatively. All anticoagulants and non-steroidal anti-inflammatory drugs (NSAID) registered in Sweden as per April 2010 were included. We estimated the optimal time of discontinuing these drugs based on their mechanisms of action, pharmacokinetic data, as well as coagulation and platelet function tests. This guideline relates to major surgery with high bleeding risk.
