Splenic injury associated with colonoscopy is a rare but potentially fatal complication. The injury mechanism consists of direct trauma by traction in the splenocolic ligament. Symptoms usually appear within a few hours to a day after the colonoscopy. Patients with suspected splenic injury should immediately undergo CT. Most splenic injuries are treated conservatively with antibiotics, intravenous fluids, blood transfusion and hemodynamic monitoring. The indication for surgery is continuing hemodynamic instability.
