TRALI is a transfusion-related lung complication which today accounts for the majority of transfusion-related deaths. It is characterized by acute lung edema where 70% of sufferers require mechanical ventilation and carries a high mortality. The pathophysiology is largely unknown, but several different pathways to TRALI development have been proposed. A sizable proportion of cases seem to be provoked by antibodies of donor origin which react with leukocyte antigens in the recipient and cause massive inflammation in the pulmonary endothelium. Female donors, and especially multiparous women, test positive for such antibodies more often than male donors leading to exclusion of female donors in many countries. Nevertheless for a significant number of TRALI-cases, no antibodies are found. Much of TRALI-research is hampered by lacking clinical recognition. Thus, a more widespread knowledge of this life-threatening disease may help improve diagnosis and treatment, and ultimately prevent a non-negligible number of deaths.
