Sammanfattning

Atrial fibrillation (AF) is a common arrhythmia among open-heart surgery patients. According to international guidelines, symptomatic and selected asymptomatic patients should be offered surgical ablation of AF. The gold standard in AF surgery is the Cox Maze III procedure, in which both atria are treated in a specified pattern (“cut-and-sew”) in order to prevent AF wavelets to develop. After one year, over 90% of Cox Maze III-treated patients are free of AF. The development in ablation technology has introduced several new surgical ablation catheters and simplified lesion patterns have been suggested, however not reaching the high success rate of the Cox Maze III procedure. The Swedish Arrhythmia Surgery Working Group has therefore reached a consensus of surgical treatment of concomitant AF in open-heart surgery patients. The consensus statement emphasizes adherence to the lesion pattern in the Cox Maze III procedure, and the use of biatrial lesions in non-paroxysmal AF.
